Health Insurance
Coverage for Employee Only is $15.50/week thru payroll deduction. Employees may also carry dependent health coverage thru payroll deduction.
Highlights of the plan for employees seeing CIGNA Open Access Plus Providers includes $15 Co-pay for Primary Care Physicians, $30 Co-pay for Specialists, and a $300 deductible with $3,000 maximum out of pocket. Employees not residing within the network area have the same benefits as if care were provided by a network provider.
Pre existing conditions require a one year waiting period or proof of prior insurance. |
Drug Card
The prescription card is available if health insurance is elected. Most national chains and major independent pharmacies participate in the Pharmacare network. Employee Co-pays for prescription drugs are $10 for generic, $30, $45 or $65 for name-brand. |
Dental Coverage
Depending on the dental plan elected, coverage for Employee Only is $2.00 or $3.79 per week. Preventive care is covered at either 80% or 100% (depending on plan choosen) of reasonable and customary charges with no deductible - one visit each six month period.
Other covered services are paid at 80% or 50% of reasonable and customary charges after a $50 annual deductible. This deductible is per person with a family maximum of $150. Benefits for major procedures are excluded for a person uhe or she is covered for dental benefits for twelve consecutive months.
The maximum calendar year dental benefit per covered person is $500 or $1,000. Employees may carry dependent dental coverage through payroll deduction. With the enhanced dental plan, you also receive a $1,500 lifetime orthodontic allowance. |
Vision Coverage
The basic vision plan allows for one routine eye exam each year with a $15 co-pay and costs the employee nothing (dependents may be added to this plan at no charge).
The enhanced vision plan for Employee only coverage is $1.49 per week and after a $15.00 co-pay pays 100% of a routine eye exam and either contacts or single vision lense glasses. Some restrictions apply to both plans. |
Life Insurance
There are no medical questions when you enroll when eligible.
- Up to $150,000 on you
- Up to $50,000 on spouse
- Up to $10,000 on each child
This optional insurance is paid for by employee through Payroll Deduction.

| Optional Disability Plan
Coverage begins when Salary Continuation Benefits end. The plan extends your $250.00 weekly benefit up until the end of the first year of your disibility and then pays 60% of earnings, capped at $5,000 per month up to addition 2 years of disibility. This plan integrates with Social Security Benefits (if Applicable). This is optional insurance and is paid for by employee through deduction.

Direct Deposit
Direct deposit is mandatory. Your pay can be directly deposited into your checking/savings account or your pay can be loaded to your Comdata card. Allow 3 to 4 weeks for setup. |
Vacation Pay
All full-time drivers affected by this agreement who have been in continuos employment will receive paid vacation as follows:
- After 1 year - 1 week
- After 2 years - 2 weeks
- After 10 years - 3 weeks
Vacation pay will be computed on driver's weekly average pay for the preceding six (6) months. All vacations must be taken within the year prior to the anniversary date, or vacation will be lost. Vacation pay is not pro-rated between anniversary dates; therefore, you must work up to the anniversary date each year to earn vacation pay. Drivers hired before August 1, 1988 will receive four(4) weeks vacation after 20 years. Vacation runs on the drivers anniversary year-not the calendar year.

Continuation of Benefits
If a driver (who is not terminated) is unable to work and is notified by the Company in writing that he/she has been put on inactive status, his life, disability and health, dental and vision insurance provided by the company will continue for up to three (3) months.
Examples of inactive status include but are not limited to the following:
- Workers Compensation - Disability
- Accident or Sickness
While on inactive status, the driver must pay for any insurance premiums he/she has on payroll deduction, i.e. health, dental, optional insurance. If premiums are not paid, insurance coverage will be cancelled. While on inactive status, the driver will not be eligible for holiday or funeral pay, nor will he earn vacation pay for the period he/she is on inactive status. |
Funeral Leave
Three (3) days paid funeral leave will be permitted for death in the immediate family. Immediate family shall consist of Spouse, Child, Mother, Father, Step-child (who is a member of household), Brother or Sister. The rate of pay will be $40.00 per day.
It is understood that the employee may need more than three days of leave and additional unpaid leave may be granted, based upon the individual situation. |
Jury Duty
Road drivers who are summoned for jury duty will be paid by Epes the difference between $40/day and the amount received from the court. |
401(k) Retirement Savings Plan
After six (6) months of continuous employment, full-time employees are eligible to join the EPES 401(K) Retirement Plan.
Employees may contribute from 1% to 15% of annual earnings. The Company will match 50% of the first 4% of the employees contributions. Entry dates into the plan are the first of each quarter.
Company contributions are 100% vested after three (3) years of employment. There is also a loan program with our 401 (k) plan. |
Uniform Program
Epes will pay full price of two(2) sets of uniforms or a total of (4) items. After one year of employment and on each anniversary, Epes will buytwo(2) sets of uniforms. |
| Safety Shoe Program
Upon employment, all drivers are encouraged to participate in the safety shoe program. Epes will pay the first $50.00 one time each year for a pair of safety shoes. |
Scholarship Fund
Epes Carriers makes available a scholarship award to deserving children of Epes Carriers Subsidiary employees. Application is made in essay form and student must be a recent high school graduate |
| Top Achiever Program
Epes will honor a select number of full-time drivers who must represent SUPERIOR STANDARDS of PERFORMANCE in every area of their responsibility.
To be eligible, a driver must meet the following criteria:
- Employed from January 1 to December 31
- Run a minimum of 100,000 miles
- No Service Failures
- No preventable accidents
- No injuries resulting in lost time from work
- No Log Policy Violations
- Acts of omission - Level B
- Hours of service - Level 4
- Falsification - Level 3
- No violations of Personal Conduct Rules and Regulations.

Appreciation Award
Recognizes those drivers who go above and beyond? their normal daily responsibilities. Presented with a letter of accommodation and a $50.00 gift certificate from Walmart. |

Recognizes highway heroes and/or good deeds rendered to fellow drivers and/or the general motoring public. Presentations will be at quarterly safety meetings. |

| Safe Driving Awards
Recognizes safe driving for those drivers who are not involved in a preventable and/or chargeable accident during their anniversary year. Certificates, patches for uniforms and decals for your assigned tractor are issued.

Rider Program
A Year Around program designed for drivers and passengers that meet requirements.
NOTE: May have only one passenger at a time.
Driver Requirements:

Transflo - Truck stop scanning allows you to scan your paperwork after delivery of your load each day at selected truck stops. No more missed paydays or extra cost associated with submitting paperwork to the company. |
| Roadside Inspection Award
Epes will give $15.00 cash for all roadside inspections with no violations that are turned into the driver manager. |
HazMat Endorsement Fee Reimbursement
Epes Transport will reimburse the cost incurred for obtaining/renewing a hazardous material endorsement for a CDL license. |
Holiday Pay
| After 90 days of employment, full-time road drivers will receive $45.00 holiday pay for the following holidays: |
